- Example: If your Social Security Number is 123-45-6789, you will enter your username as 123456789
Password: The last four digits of your Social Security Number, along with the last two digits of the year you were born will be your password.
- Example: If your Social Security Number is 123-45-6789, and the year you were born was 1985, your password will be 678985
