How to login to complete your enrollment

What is your username?
Your username is a six-digit number. Find your fellowship number on the back of your UPCI Fellowship Card. If your fellowship number is NOT six digits you will need to add zeros to the front of your fellowship number until it reaches six digits.

Example #1: If your fellowship number is 54 you will add as many zeros as needed to make it six digits. Your username would be 000054.
- Example #2: If your fellowship number is 109234, your username is already six digits, and you would not need to add any leading zeros. You will enter 109234 as your username.
What is your (PIN) password?
Your password is eight digits. It is the last four digits of your username, plus your full birth year.
- Example #1: If your username is 000054 and the year you were born is 1975, your password will be 00541975
- Example #2: If your username is 109234 and the year you were born is 1975. Your password will be 92341975.
